Job Summary:

The Patient Transition Specialist is a professional whose duties and responsibilities are to support patients and their family as they move their care from our pediatric clinic to adult care at another institution. This would include, but is not limited to:
  • Creating and maintaining resource lists and internal tracking documents for adult providers,
  • Collaborating with TRUE team members to compose transition summary letters for patients and providers,
  • Meeting with patients and families during/after clinic appointments, to identify needs, coordinate, and assist in transitioning to adult care, following up as needed,
  • Identify and establish working relationships with adult care programs across the state and region, ensuring they are affirming and safe spaces for our patients and maintaining accurate information, and other coordination tasks as needed regarding patient's transition needs.

Work Location:

Onsite – this role is expected to work onsite and is located in Aurora, CO.

Qualifications:

Minimum Qualifications:
  • Bachelor's degree in any field.
  • 1-2 years of experience with care coordination, patient/community outreach, and/or scheduling.
  • 1-2 years of experience in a clinical setting.
    • Substitution: A combination of education and related technical/paraprofessional experience may be substituted for the bachelor's degree on a year-for-year basis.

Applicants must meet minimum qualifications at the time of hire.

Preferred Qualifications:
  • 2+ years of experience working in a clinical setting.
  • Experience working with adolescents and/or young adults.
  • Experience with Epic EMR system.
  • Experience working with transgender and gender-diverse youth.

Knowledge, Skills, and Abilities:
  • Critical thinking: Assessing and evaluating individual patients' needs to determine the most suitable solutions based on various factors such as location, insurance, co-morbidity, neuro-divergence, etc.
  • Utilize analytical, evaluative, and interpretive skills to understand and address the needs of individual patients and evaluate adult programs effectively.
  • Ability to apply expertise gained from experience or education to effectively communicate with adult programs and ensure comprehensive information gathering before, during, or after transitions.
  • Ability to establish and maintain effective working relationships with employees at all levels throughout and outside the institution.
  • Outstanding customer service skills.
  • Demonstrated commitment and leadership ability to advance diversity and inclusion.
  • Ability to communicate effectively, both in writing and orally.
  • Outgoing, organized, flexible, detail-oriented, and able to work independently to serve our families.

Anticipated Pay Range:

The starting hiring range for this position has been established as $66,537 to $84,635.

The above hiring range represents the University's good faith and reasonable estimate of the range of possible compensation at the time of posting. This position may be eligible for overtime compensation, depending on the level.
